Case description Toula is a 54-year-old woman who was born in Greece. She now lives in Coburg, Victoria, with her husband, two teenage children and her elderly father. Toula works full-time as a receptionist in a primary school. Over the past few months, she has noticed more frequent urination, nocturia and lethargy. Her weight has steadily increased over the past 10 years. Toula had gestational diabetes during her first pregnancy, which was 20 years ago. Her recent blood test and clinical review reveals a new diagnosis of type 2 diabetes, hypertension and hypercholesterolaemia. At her most recent GP consultation the findings of her clinical examination and pathology results were: BMI 31a Blood pressure 155/90 Triglycerides 2.4 mmol/L, Total cholesterol 6.5 mmol/L LDL cholesterol 4.5 mmol/L HDL cholesterol 0.8 mmol/L Fasting plasma glucose 12.7mmol/L HbA1c 67 mmol/mol; 8.3% Toula’s mother had insulin-treated type 2 diabetes, and she died from a cerebrovascular accident (stroke) five years ago. Task description For this assessment task you are required to discuss a clinical pathway suitable for Toula to manage hyperglycaemia and her other conditions of hypertension and dyslipidaemia. Include in your discussion the overall management goals and rationale for commencing glucose lowering medication for someone newly diagnosed with type 2 diabetes. Assessment Information 2 Explore and discuss the recommended lifestyle changes Toula and her family could make to facilitate optimal self-management of her newly diagnosed diabetes, including monitoring, nutrition and physical activity. Your discussion should incorporate the role of the diabetes educator in supporting Toula to self-manage her diabetes in the context of an interdisciplinary team
