Context

This is an individual task. This assessment (2B) is a follow up task of the Assessment Task 2A, where you prepared a plan for an information sheet on a medical condition for clinicians.

Being a nurse, you are expected to communicate with clinicians (Nurses, GPs, specialists or other allied health professionals) about your patient’s medical condition(s) on a regular basis. This assessment task will help you to gain skills about what types of information you should include in an information sheet when communicating with a clinician.

Task Instructions

To complete this assessment task, you must develop an information sheet on a medical condition for clinicians (Nurses, GPs, specialists or other allied health professionals), which includes the following:
 
The intended audience for the information sheet is a clinician, therefore the information needs to be credible, up-to-date and sourced from evidence-based clinical medicine and applicable to the Australian context.
 
You should demonstrate high level of knowledge of medical terminology and advanced concepts of pathophysiological and physiological principles.

Task Instructions

To complete this assessment task, you must follow these steps:

1. You will use the same medical condition that has been used in your Assessment 2A.

2. Conduct further research on the condition chosen in Assessment 2A and the bodily systems it impacts.

3. The information sheet should include information about:

•Clear identification of the condition chosen

•Definition of the condition

•Signs and symptoms

•Pathogenesis

•Prevalence

•Burden on resources / the community

•Risk factors

•Investigations and treatments

•Outcomes / complications

•Information on investigations and treatments based on the latest scientific evidence. You can also include any screening tests and links to other helpful resources.

4. Explain the pathophysiological process using relevant medical terminologies/definitions. You should pay particular attention to high standards of health literacy and how the information is presented in a meaningful and engaging way to capture the clinician’s attention.

5. Use suitable images which explain or support a concept you are addressing.

Please note that

•You are required to present your own original work using multiple academic references from academic books, journals and other credible sources. 

•You should present your work in the form of an information leaflet, including text, images and other visual aids.

•Academic references are to be included on a separate page using APA guidelines.
