Section 1: Check developmental status
You are required to report on how you engaged with the client and their carer, family or significant others to check the client’s developmental status prior to delivering services
Client:
Write a summary of this person with details such as age and background.
Describe the observations and/or questioning you used with the client to get information about their development status. For example, you could list the questions you asked or what observing the client told you about them. Describe any applicable workplace procedures.
List other documented information you were able to access to inform your assessment of the client development status. Briefly describe what information this gave you about the client’s developmental status.
Describe the information you received from the client’s carer/s, family or significant others to inform your assessment of the client development status. If there were no other persons involved indicate what information it may be possible to get from carer/s, family or significant others to inform the assessment.
Describe how you used your knowledge of lifespan development theories to clarify the client’s development status.
Section 2: Developmental issues
You are required to report on the client’s developmental issues.
Client
To assist with the identification of development issues, describe the appearance and/or behaviour of the client and outline the factors that you consider may have impacted on the appearance and behaviour of the client?
Explain potential risk factors associated with developmental issues for this client.
Describe the proposed services for this client.
As it important to make sure that the services are suitable prior to being delivered, explain why the services being proposed are suitable?
At times you may have concerns about a client’s developmental status. Describe your workplace procedures regarding this. If you did have concerns about this client, explain who you consulted with and the outcomes.
Describe signs of potentially serious issues with a client. This may or may not have occurred with this client. If it did, describe the signs you identified. If not, describe potential signs you may recognise. Describe your workplace procedures regarding referral of potentially serious issues.
Describe your workplace procedures for documenting developmental issues. Include your record regarding this client’s developmental status with this portfolio.
Section 3: Specific issues
You are required to report on how you checked for and responded appropriately to specific issues.
Client
With regards to this client, were there any developmental issues that needed to be reported. If not, describe an example where this would be the case and your workplace procedures.
Were there any indicators of any other issues for this client. If so, describe them. If not, describe example indicators of other issues.
Describe your workplace procedures for reporting specific issues. This should also include mandatory reporting requirements.
Explain the importance of reporting accurately and with the required level of detail. Make sure that all the documents you
submit (including this Portfolio) show that you can report accurately and with the level of detail required.
